All patients will need the following three forms. You can download them here and fill them out in advance of your first visit to save time.

• Confidential Patient History
• Notice Of Privacy Practices
• Informed Consent

If the condition for which you are seeking treatment is the result of a motor vehicle accident, you will also need to fill out the following:

• NYS Motor Vehicle Insurance Information
• NYS Motor Vehicle No-Fault Assignment of Benefits

If you have health insurance with out-of-network benefits, your provider may reimburse you for the cost of the treatment. Take a look at the following:
