New Patients
Please arrive 15 minutes early for your first
appointment to fill out the necessary paperwork or download the forms and fill them out in advance to save time.
Contraindications for Treatment
Acupuncture is not advised if you are altered by
drugs or alcohol, or are excessively fatigued or
hungry. Be sure to eat regular meals the day of
treatment, and continue to take any prescription
medications as directed by your physician. A. M.
Acupuncture reserves the right to refuse treatment
based on such criteria.
Payment Methods
Payment is due at the time of service. For your
convenience, payment can be made by cash, check, or
credit card.
Health Insurance
Some insurance is accepted. Please call for details.
Cancellation Policy
If you need to change or cancel an appointment for any reason, 24-hour advance notice is requested. Changed, cancelled or missed appointments without such notice will be billed
at full fee.
Confidentiality
Patient confidentiality will be strictly maintained at all times as required by HIPPA laws.
