New Patients
Please arrive 15 minutes early for your first
appointment to fill out the necessary paperwork or download the forms
and fill them out in advance to save time.
Contraindications for Treatment
Acupuncture is not advised if you are altered by
drugs or alcohol, or are excessively fatigued or
hungry. Be sure to eat regular meals the day of
treatment, and continue to take any prescription
medications as directed by your physician. A. M.
Acupuncture reserves the right to refuse treatment
based on such criteria.

Payment Methods
Payment is due at the time of service. For your
convenience, payment can be made by cash, check, or
credit card.
Health Insurance
With some exceptions, A. M. Acupuncture is a fee-for-service practice.
Patients with out-of-network coverage are provided
with appropriate paperwork to submit for reimbursement from their
insurance carriers. Those without acupuncture
coverage may qualify for an Affinity Discount. Please inquire about your
particular plan and see Instructions for Submitting Insurance Claims for Reimbursement.
- If your condition is the result of a car accident...
In New York State, if you have suffered a personal injury in a motor vehicle accident, Article 51 of the Insurance Law (the No-Fault Statute) requires the auto insurance carrier to pay for treatments. You will need to fill out some additional forms.
Cancellation Policy
If you need to cancel an appointment for any reason, the courtesy of 24-hour advance notice is requested.
Missed appointments without notice will be assessed
at full fee.Confidentiality
Patient confidentiality will be strictly maintained at all times as required by law.
